Avastin May Help Some With Ovarian Cancer: Studies (HealthDay)

WEDNESDAY, Dec. 28 (HealthDay News) -- Two new studies suggest that the drug Avastin may lengthen progression-free survival by about four months for women with ovarian cancer.

What isn't clear yet is whether adding Avastin (bevacizumab) will make a difference in overall survival. One study suggested it would, while the other study didn't find a difference in overall survival between two treatment groups.

"The bottom-line results are a 28 percent reduction in disease progression with patients on continued bevacizumab compared to the standard treatment group. There was a 3.8 month median difference in progression-free survival," said Dr. Robert Burger, author of one of the studies and director of the Women's Cancer Center at Fox Chase Cancer Center in Philadelphia.

"We still have to fine-tune or optimize this sort of new paradigm of treatment. This regimen [chemotherapy plus continued Avastin] could be considered a frontline option for patients with advanced ovarian cancer. However, there are some risks to the treatment, and we haven't shown an impact on overall survival. But, in the European trial, they saw an overall survival benefit," Burger added.

"I think the difficult reality is that these studies are not clear-cut," said Dr. Len Lichtenfeld, deputy chief medical officer for the American Cancer Society. "The improvements in progression are modest and appear to be greater for women who have worse disease. What this ultimately means is that it's absolutely important for doctors treating women with advanced ovarian cancer to read these studies very carefully so they know who may benefit and the risks of treatment, and to counsel their patients very carefully before moving forward with Avastin treatment," Lichtenfeld explained.

"This is not a case where treatment can be initiated in every woman. The benefits for many women are modest at best," he added.

Results of both studies are published in the Dec. 29 issue of the New England Journal of Medicine. The studies were funded by Roche and Genentech (a Roche-owned company). Burger said Genentech played no role in the study design or analysis.

Burger's study included almost 1,900 women with newly diagnosed stage 3 or stage 4 epithelial ovarian cancer. All of the women had undergone surgery to remove as much cancer as possible. They were then randomly assigned to one of three treatment groups: standard treatment including paclitaxel and carboplatin chemotherapy; the Avastin-initiation group that included the standard chemotherapy and Avastin for the second through sixth treatment cycle; and the Avastin maintenance group that included chemotherapy plus Avastin for up to the 22nd treatment cycle.

The average progression-free survival was 10.3 months for the standard group, 11.2 months for the Avastin-initiation group and just over 14 months for the Avastin-maintenance group. There was no difference in overall survival for the three groups.

The European study included more than 1,500 women with varying types of ovarian cancer. Most had epithelial ovarian cancer, but the severity ranged from early-stage disease to stage 4. Seventy percent of the women enrolled in this study had stage 3 or stage 4 cancer.

The women were randomly assigned to receive either standard chemotherapy treatment or chemotherapy plus Avastin for up to 12 cycles of chemotherapy. The dose of Avastin used in this trial was half of what was used in Burger's trial.

Progression-free survival was 20.3 months for the standard group and 21.8 months for the standard therapy plus Avastin. In women who were considered at high risk for progression, the progression-free survival was 14.5 months on standard therapy and 18.1 months with the addition of Avastin. Overall survival in high-risk women was 28.8 months for women on standard chemotherapy and 36.6 months for standard therapy plus Avastin.

High blood pressure is a common side effect of Avastin, but Burger said that in most cases, it can be controlled with blood pressure medication. Another serious side effect associated with Avastin is bowel perforation, which means a hole opens in the bowel wall. This side effect occurred in fewer than 3 percent of women treated, but occurred twice as much in women taking Avastin.

Published reports peg the cost of the drug at somewhere between $4,000 and $9,000 a month, depending on co-pay assistance that's available from Genentech, Avastin's manufacturer. In a previous study, another research group looked at the cost effectiveness of treatment with Avastin in Burger's trial and found the standard treatment arm of the trial cost $2.5 million. Treating the Avastin-initiation group cost $21.4 million and the Avastin-maintenance group cost $78.3 million, according to the study, which was published in the March 7 issue of the Journal of Clinical Oncology. And, most of those costs came directly from the cost of the medication. Those researchers concluded that Avastin was not a cost-effective medication.

"We don't want cost to be a deciding factor, but for many women with ovarian cancer, cost may be a significant factor, and it needs to be weighed in the equation," Lichtenfeld said.

Avastin isn't approved in the United States for the treatment of ovarian cancer, so some insurance companies may balk at paying for it. Plus, the U.S. Food and Drug Administration pulled Avastin's approval for the treatment of metastatic breast cancer in November because the FDA found the risks of the drug outweighed its modest benefits. Avastin is still approved for the treatment of some types of colon, lung, kidney and brain cancer in the United States.

However, the drug was just approved by the European Union for use in ovarian cancer in combination with chemotherapy.

Japan also had a heroic explorer dashing to the South Pole 100 years ago ? and he did it on a shoestring

FOR a few weeks in January 1912, Antarctica was teeming with explorers. Roald Amundsen and his Norwegian party had reached the South Pole on 14 December and were speeding back to the coast. On 17 January, Robert Scott and the men of the British Antarctic expedition had arrived at the pole to find they had been beaten to it. Dejected, they began to retrace their steps in what turned out to be their final journey. Just then, a third man with polar aspirations arrived on the scene. Nobu Shirase was a little late but no less determined to cover himself in glory.

In the story of the race to the South Pole, Shirase is the invisible man. A Japanese explorer, his part in one of the greatest adventure stories of the 20th century is hardly known outside his own country. Yet as Scott was nearing the pole and with the world still unaware of Amundsen's triumph, Shirase and the Japanese Antarctic expedition sailed into Antarctica's Bay of Whales in the smallest ship ever to try its luck in these perilous waters. On 19 January 1912, the little wooden schooner sailed up to the edge of the Ross ice shelf and left Shirase and his men to scale the immense wall of ice ready for a daring dash south.

Lieutenant Shirase was a middle-aged army reservist who since boyhood had dreamed of becoming a polar explorer. In Japan, the very idea was startling. When Shirase was born, people were forbidden to leave the country on pain of death. The overthrow of the ruling dynasty in 1868 brought modernisation and new ideas, but they didn't extend as far as polar exploration. Undeterred, Shirase toughened himself up in readiness. He didn't drink or smoke. He spurned the warmth of a fire in winter and refused hot food and drinks. Like Amundsen, he initially set his sights on the North Pole. But after the American Robert Peary claimed to have reached it in 1909, both men hastily revamped their plans. Instead, like Scott, they would aim for the last big prize: the South Pole.

In January 1910, Shirase put his plans before the elected officials of Japan's Imperial Diet. "Within three years," he told the assembled politicians, "I vow to raise our Japanese imperial flag at the South Pole." For many, the question wasn't could he do it but what was the point? It wasn't just about being first to the pole, Shirase said. It was also about science.

The agenda for would-be Antarctic explorers had been set 15 years earlier by the International Geographical Congress. As the last unknown continent, the congress declared, the Antarctic offered the chance to add to knowledge in almost every branch of science. So, like the British, Shirase presented his expedition as a quest for knowledge rather than a bid for personal glory. He would bring back rocks and fossils, make meteorological measurements and explore unknown parts of the continent.

The response was cool. Neither the government nor the public had much appetite for such a venture and the press poked fun at the whole idea. Shirase struggled both to raise funds and to find scientists to accompany him. His supporters accused the nation's scientists of being too keen on home comforts and too cowardly to risk their lives for science. Who needed them anyway, argued one. It didn't take an Einstein to collect rocks or jot down the temperature or wind speed.

A few months later, Japan's former prime minister Shigenobu Okuma came to Shirase's rescue. With Okuma's backing, Shirase scraped together enough money to buy and equip a small schooner, quickly renamed the Kainan Maru, or "Southern Pioneer". He eventually acquired a scientist, too, albeit not one known in the usual scientific circles.

At the end of November 1910, the Kainan Maru finally left Tokyo with 27 men and 28 Siberian dogs on board. Before leaving, Shirase confidently outlined his plans to the press. He would reach Antarctica in February, during the southern summer.

Then, like Amundsen and Scott, he would spend the winter exploring and preparing for his push to the pole the following spring: "On 15 September, when the winter will have ended, the party will proceed to the pole." His men, he proclaimed, would travel more than 1400 kilometres over the ice in 155 days and "return to the rendezvous by the latter part of February 1912".

Things didn't go according to plan. The difficulty raising funds had already delayed the expedition. Bad weather delayed it further. The storm-battered Kainan Maru didn't reach New Zealand until 8 February; Amundsen and Scott had already been in Antarctica a month and were now preparing for winter.

When the ship stopped at Wellington to take on supplies, New Zealand's reporters flocked to the quayside. They were astonished. At 200 tonnes and 30 metres long, the "strange little three-masted vessel" was half the size of Amundsen's ship, Fram, and a third the size of Scott's Terra Nova. True, the hull was reinforced with extra planking and iron plate, but the ship had only the feeblest engine to help force its way through ice. Could it really survive in the unforgiving Southern Ocean?

Jazz musician Sam Rivers dies from pneumonia at 88 (AP)

ORLANDO, Fla. ? Sam Rivers, an internationally-known jazz musician who played with Miles Davis and Dizzy Gillespie, has died. He was 88.

Monique Rivers Williams says her father died Monday night from pneumonia.

The Oklahoma native was a saxophonist, flutist and composer.

He started his career in Boston, where he performed with Herb Pomeroy's big band in an ensemble that included future music producer Quincy Jones. In 1964, he moved to New York and was hired by Davis. He played with a diverse group of musicians there that included Gillespie, T-Bone Walker and John Lee Hooker.

He moved to Orlando in the early 1990s and regularly played with a group of jazz musicians whose day jobs were at Walt Disney World.

Plans are being made for a public memorial concert.

State?s top juniors begin practices for all-star football game

We?ll be providing updates all week from the 2011 Junior Bowl, which features 90 of the top college football prospects that are high school juniors from across the state of Georgia.? Friday?s game will be played at 7 p.m. at Grady High School and televised on ESPN3.com

Every year at the Junior Bowl, there are unknown players who ?come out of nowhere? to register on the recruiting radar after outstanding performances in practice against blue-chip prospects who already boast early scholarship offers.

Last year, it was Raphael Kirby who was ?discovered? at the Junior Bowl. Kirby didn?t have any scholarship offers at the time but turned heads with his stellar play at linebacker. Word quickly spread about Kirby, the Stephenson High School standout who later committed to Miami out of his eventual 38 scholarship offers.

A leading candidate for this year?s biggest discovery is Milton High School?s Carl Lawson, who dominated Tuesday?s practices at defensive end. He has no early offers, and was not an automatic selection for the game but won over RisingSeniors.com co-founder Joe Burns when he attended a Milton game this past season. The 6-foot-3, 250-pounder proved worthy of the selection during Tuesday?s practices.

We?d have to name Lawson the overall MVP from Tuesday?s sessions. ?He had a great day; he?s explosive and has great hands ? he showed everything in his arsenal that he needed to show,? said Rusty Mansell, National Recruiting Analyst for 247Sports. ?I think he can play defensive end in a 4-3 defense. He?s strong and he?s got every move ? I?ve heard a lot of people talk about him. After seeing him today, I think he?s going to be getting some offers soon.?

East Defense, Biggest surprise ? Nigel Bowden, the 6-foot-2, 226-pound linebacker from Central Macon. ?It was amazing how fast he runs at 225 pounds,? Mansell said. ?He covered the field from sideline-to-sideline with a lot of speed. That?s what you want out of a linebacker. He plays with good instincts.? Bowden, who has a 4.0 GPA, has an early offer from Vanderbilt.

East Offense, MVP ? Alvin Kamara, RB, Norcross High School: ?He can fly. He?s so fast, and can catch the ball. He?s fluid, and everything looks easy to him. He?s got the tools you want to see at running back.? The 5-foot-9, 165-pounder has an early offer from Syracuse.

East Offense, Biggest surprise ? JP Vonashek, the 6-6, 265-pounder who recently transferred from Glynn Academy to Camden County. He has no early offers. ?I didn?t really know much about him but he?s definitely a high-level D1 prospect,? Mansell said. ?He?s tall with a great frame. He has a great punch when he?s blocking and moves well with his feet. He can pass block, run block, and has a great knee bend for being that tall.?

Among musicians, ?flat? is a word you don?t want to hear. But to the industry,?flat?is truly music to everyone's ears.

Soundscan?s sales year doesn?t end until Jan. 1, but current year-to-year comparisons have album sales for 2011 ahead of comparable 2010 business.

OK, it's only by a whisker of 1 percent. But it is welcome news in an industry where album sales for 2010 were down 13 percent from 2009. And prior to that they'd been down an average of 8 percent every year through the 2000s, suggesting an incredible shrinking music business.

This year reversed the trend, but there is a cloud buried in the silver lining: the success stories of 2011 will not be easy to replicate.

The year?s two biggest albums were Adele?s ?21? and Michael Buble?s ?Christmas? -- and even the most copycat-prone execs aren?t foolish enough to start looking for pleasingly plump Englishwomen or Sinatra-bred carolers to sign.

Regardless of how applicable the lessons might be, here?s our look back at what worked and what didn?t in 2011:

RETRO ROCKS ? IF YOU DON?T CALL IT RETRO

Artists from Adele to the Black Keys thrived by recalling good old days for oldsters while seeming utterly contemporary to kids who'd rebel at the word ?throwback.?

At last tally, Adele's ?21? had sold 5,281,000; with two sales weeks yet to be reported, the blockbuster should finish out the year a little shy of 6 million. (The only release in the past few years to cross the 6 million mark was Taylor Swift?s ?Fearless.?) Moreover, she sold an additional 750,000 copies this year of her previous album, ?19.?

Adele is a singles artist, too. ?Rolling in the Deep? has sold 5,665,000 downloads, followed by ?Someone Like You? with 3,352,000, ?Set Fire to the Rain? with 963,000, and ?Rumour Has It? with 551,000.

On a smaller but louder end of the spectrum, the Black Keys put the lie to ?rock is dead? theories -- again -- by moving 426,000 copies this year of their two-year-old ?Brothers." Their brand new ??El Camino,? whose nostalgic aspects lean more toward glam-rock than neo-blues, has sold an impressive 293,000 units in two weeks.

BET ON THE RIGHT CAROLER

Everyone guessed there?d be a big Christmas album this year. Almost everyone guessed it?d be Justin Bieber?s. But Buble's sold 1,964,000 of his holiday CD, versus Bieber's 1,003,000.

We should have seen it coming, since Buble?s previous album quietly sold 2 million-plus. Despite his crooner image, Buble?s holiday set wisely had something for everyone, whereas Bieber?s had something to annoy just about everyone outside his core.

"Christmas" currently stands at No. 3 on the list of 2011?s bestsellers, and will surpass Lady Gaga to land at No.

Berri threatens to question cabinet?s performance if oil bill stalled

Speaker Nabih Berri threatened to question the Lebanese cabinet?s performance if it does not issue relevant decrees to put in action the oil exploration bill, which was approved by parliament in 2010.

?I will call on the parliament to convene and carry out its role in questioning [the cabinet?s performance], even if the parties concerned are my allies and friends,? Berri said in an interview with As-Safir newspaper published on Tuesday.

He added that ?Prime Minister Najib Mikati and Energy Minister Gebran Bassil voiced commitment to issue the decrees [to implement the oil projects].?

?I will not allow this commitment to be broken,? the speaker said.

?Cyprus and Israel began the procedures to launch oil exploration while we keep wasting time? I will no longer bear this issue.?

Lebanese Parliament in August 2010 passed an oil exploration bill, which calls for the establishment of a treasury and a committee to oversee exploration and drilling off Lebanon?s coast.

South Korean president urges stability in NKorea (AP)

SEOUL, South Korea ? South Korean President Lee Myung-bak is urging stability in North Korea and says his country's front-line troops remain on alert.

South Korea put its troops on high alert Monday after the death of Kim Jong Il. The presidential Blue House says Lee made the comments Thursday in a meeting with lawmakers.

All signs out of North Korea suggest the transition of power to Kim Jong Il's son is going smoothly. U.S. and South Korean officials are seeing no unusual troop movements and no buildup around the heavily fortified border.

Pentagon press secretary George Little said that as of Wednesday the U.S. military has not seen any unusual military movements by the North Koreans.
