Prostate Cancer Clinical Test And Statistics

Larger-volume tumors of the prostate are common among older men. Available tests for the early detection of cancer have limited specificity, which necessitates a relatively high biopsy rate. The positive predictive value of combined digital rectal examination and PSA measurement has been defined, but the negative predictive value is less clear. Measurement of PSA is the most sensitive noninvasive test for prostate cancer. However, digital rectal examination detects cancer that would otherwise be missed by PSA measurement.

Clinical trials serve as one way of finding prostate cancer cures. All prostate cancer medications must take pass the three phases required to gain approval from the Food and Drug Administration.

Phase I of the clinical trials test the safety of a new drug. The second clinical phase is designed to determine how the proposed new prostate cancer treatment works. Patients are given the drug in high doses during this phase. The patients are watched to see what effect the test drug has on their prostate cancer. The final phase of clinical trial testing pits test medications against standard treatments. A control group is given dosages of the test drug while a second group uses standard methods of medicine-with the effects documented

The purpose of the trial is to test the safety and effectiveness of the technique, called high intensity focused ultrasound (HIFU), as the initial treatment in men with newly-diagnosed, localized prostate cancer.

Undiagnosed prostate cancer is highly prevalent, especially among older men [42-49]. Although many of these cancers may be considered incidental, evidence suggests that consideration of screening is warranted because earlier diagnosis of clinically significant cancers often has the potential to improve outcome [10, 12]. In fact, prostate cancer is so common among older men that selecting subpopulations for screening on the basis of risk factors other than age, such as race or family history, would not be necessary if screening and treatment strategies that favorably affect outcome were available.

However, evidence also indicates that available tests for early detection have limited specificity, particularly among older men who have benign prostatic hyperplasia. Therefore, the combined use of digital rectal examination and the currently available assays for PSA results in as many as 15 of men in their 70s requiring further invasive evaluation with biopsy. Positive predictive values are 15, depending on age.

Prostate Cancer Statistics:

Prostate cancer is the most common type of cancer found in American men, other than skin cancer. Prostate cancer is the second leading cause of cancer death in men, exceeded only by lung cancer. Although men of any age can get prostate cancer, it is found most often in men over 50. In fact, more than 70 between 1973 and 1992.
One in six American men is at lifetime risk of prostate cancer. If a close relative has prostate cancer, a man?s risk of the disease more than doubles. With two relatives, his risk increases fivefold. With three close relatives, his risk is about 97 ? 50 of all new cancer cases in American men.

This year, more cases of prostate cancer in men under the age of 65 are expected than the combined number of men of ages who are victims of leukemia, Hodgkin?s disease, and brain tumors.

Prostate cancer accounts for approximately 15 of male cancer deaths. Yet, on average, only about 5% of federal cancer research dollars have been devoted to beat the disease.

It makes me want to puke.

On reading this you will have had one of two reactions ? you?ll either have nodded along because you, too, have always known what you wanted to do and discovered it early.

Or it edged you closer to despair because finding your passion is like playing hunt-the-thimble in a game rigged by a particularly perverse game master.

Guess what? Yet another post from a celebrity who effortlessly discovered their passion and went on to pursue it with great success. You could be forgiven for wondering whether you missed out on the ?passion-finding gene?. And you?re not alone.

All the evidence shows that people who know what they want are more likely to get what they want. After all, if you don?t know what you want, you will end up working for someone who does. Without your own agenda you will spend the best part of your youth and energy delivering someone else?s agenda.

What also seems true is people who do manage to know what they want possess a kind of ?inner compass? allowing them to make decisions and change their circumstances to move nearer to it.

Why then is it so rare?

There are myriad goal-setting books on the market and an equal number of goal-setting systems, each with its own fans. Much of this ignores the plain truth ? most people, when you ask them, don?t know what they want. Goals, beyond a daily to-do list, are a hazy concept for most. An even greater mass of people have not made it as far as using an effective to-do list.

For all the talk of outcome orientation there is very little of it out there. We are drowning in advice and yet, aside from a tiny minority, most people don?t set goals and this year will pass pretty much like last year.

I have always found this fascinating. There appears to be a complete disconnection between the standard self-help, personal improvement, creativity mantra (I call it ?Knowing and Going?) and the daily reality of working with clients who remain not only clueless but stuck because they are unable to work it out.

Test it for yourself. Next time you start a meeting try asking people why they are there and what they want to walk away with? Watch as they duck and run for cover. Hard to believe I know, but most folk who trundle along to meetings do not know why they are there and cannot say what they want to leave with.

If something is not yet done it comes down to one of two problems:

Either you know what you want but don?t know how to get it
Or, you don?t know what you want

It?s people with the first problem who are writing and reading all those articles and books on success and change. They more or less know what they want but need a tool, technique or advice to help them get it. These folk seem unaware they are part of a very small group. They also assume the second problem is easy ? you just think of it and then go for it. You know and go.

This is all fine, until you begin talking to people with the second problem. That?s when you find it?s a real problem and for many it strangles change at birth. There is almost nothing out there to help these folk.

Unfortunately the people writing all those self-help/productivity/creativity blogs don?t understand this. They assume all you have to do is ?work out what you want? or ?know what you want? and you can skip to the next step in their pet system. But what if you don?t? What if you can?t get past step one because nothing you think of seems to ignite your passion?

Is any of this familiar?

You know what you don?t want but struggle to be clear about what you do want.
You could do almost anything but you end up doing almost nothing because you lack the certainty to pick something.
You don?t like where you are but can?t figure out where you should be.
You know you are holding yourself back but you are not sure from what.
You have let others make the bigger decisions about your life; you suspect you might be coasting.
You carry around a vague but pressing feeling there is something else for you, if only you could know what it is.
When asked what you want, your mind goes blank or your heart starts racing. It scares you.
You?re facing a big decision and you don?t know what to do, or you have a feeling you might know but are afraid of the answer.
You?re scared of making the changes you know you could be making.
Somehow, you know your life would be different if only you could work out what you wanted.

Congratulations, you have the second problem ? you can?t ?go? because you don?t ?know?.

I?ve spent years helping clients get past this and collecting tools to help with just the first step ? working out what you really want. Last year, I put the best of them into a book, First, Know What You Want ? Why Goals Don?t Work and How to Make Them. Inside you?ll find twelve practical experiments in finding your ?inner compass?, whether you?re the analytical type or more intuitive. And some bonus tips on using these techniques with others.

Here are a couple to get you started:

1. Start with What You DON?T Want

Unless you are a highly motivated, goal-seeking machine who uses positive language all the time, the chances are you spend at least some of the time thinking about things you don?t like and don?t want. Some of us are really good at this, so good that when someone asks What do you want? it takes us by surprise and we get a bit flustered.

Make it easy for yourself and start where you are ? with what you don?t want. Pick something right now that you don?t want in your job, career, future, art, or relationship. Got something? Now ask What do I want, instead?

What you want is not always the opposite of what you don?t want:

I don?t want to breathe recycled air all day.

What do you want instead?

I?ve always wanted to work near the sea or I?d like to be hotter during the day or whatever that little ?instead? unlocks for you.

If What do you want? is the most powerful question you can ask, What do you want, instead? is probably the second most powerful question.

First you go with your natural focus ? the moan, the irritation, the thing you don?t like. Then ask yourself What do I want [pause] instead? To answer you will have to glance in some other direction, searching around until you find an alternative.

2. Stop Thinking and Come to Your Senses

Instead of using your head to tease out what you want, why not ask your senses? They have a pretty good idea of what pleases them, but they talk slowly, like your uncle who takes forever to get to the point.

Often we numb or ignore our senses because we are moving through our lives too fast and it is annoying to have to slow down and pay attention to them. It is a bit like taking a toddler for a walk. The little one keeps wanting to go in different directions and stopping to investigate interesting things.

Hurry up, hurry up you cry to your senses, pulling on the leash. It?s no wonder they stop talking to you after a while. In your senses lie some powerful clues about what you want.

Mike came to see me facing a job change and uncertain about what he wanted next. After getting nowhere for a while I asked him what his favourite smell was. He thought for a moment. ?It?s my daughter?s hair, just after she has had a bath and is having a bedtime drink.?

When I asked him how often he got to enjoy that smell and how he could enjoy it more, he began to well up. ?I?ve been so busy with work in the last few months I can?t remember the last time.?

What?s your favourite smell? was enough to help Mike get to the heart of what he wanted from work and family. Could your senses help you gain some clarity too?

What do you like to touch (or be touched by)?

What do you love to taste?

And your favourite smell? Could it contain a clue about what you want?

We have somehow confused adulthood with having less physical pleasure in our working day but it?s totally OK to devise a life that pleasures your senses (in case you were wondering).

If nothing else is working why not stop thinking and come to your senses?

In "Miss Bala," Laura(Stephanie Sigman), a 23-year old shop girl, and her friend Suzu(Lakshmi Picazo) sign up for a beauty pageant, Miss Baja California. That night instead of shopping for a dress, Suzu drags her friend along to a nightclub. Bored, Laura wanders off where she is found by a gunman who asks her questions about who is inside, gives her a little money and tells her to get lost. Well, not before Laura can get Suzu out but they become separated once the shooting starts and Laura makes it to safety out a window. The following morning Suzu is still missing and Laura does the sensible thing in going to a policeman. But instead of taking her to a police station, he takes her to Lino(Noe Hernandez), the criminal leader, who asks her what she knows before inquiring if she can drive a car. There are statistics given at the end of "Miss Bala" about how many people have died in the drug wars in northern Mexico. Now, using a beauty pageant may seem like a screwy way at first to get there but according to the literature, a beauty queen is supposed to represent her subjects and Laura does, if you mean her subjects include those trapped in between the authorities and the criminals. The movie is on neither side, as it paints the authorities as corrupt and in cold blooded fashion Lino lets the DEA know that they are not welcome in Mexico. All of which is seen through Laura's eyes as she provides a ground level view of what goes on around her for the viewer, as she becomes little more than a commodity to the criminals with her body(notice how it is described, by the way) used and abused, with little choice in her actions. Even with large amounts of money on the table, she is not really tempted, either, with the exception of a scene in an upscale dress shop. The most important lesson in all of this is to always listen to your parents.

Cemetery science: Gravestones, few dead-ends

To a geologist, a gravestone can offer information other rocks can't. One project is using gravestones to better understand how the elements, particularly acid rain, are weathering rocks around the world, and how that's changed over time.?

"It is a great place for us to collect scientific data because gravestones have got dates on them, it is not that we have a morbid fascination," said Gary Lewis, director of education and outreach for the Geological Society of America, which is in charge of the Gravestone Project.

That date of death gives a good estimate of when the stone went into the ground above the grave and began to face elements.

The wear and tear on the stone that follows can be caused by freezing and thawing temperatures, lawn care machinery and rain made acidic by pollutants it has picked up in its course through the atmosphere.

"What we are trying to do is not just look at damage by acid rain, but we are trying to see how acid rain has changed over time," Lewis said.

The Gravestone Project recruits volunteers around the world to head into cemeteries where they use calipers to measure the width of a stone at five points along its sides and at its top. If a stone has lead letters on it, volunteers measure how much the stone has worn away from the lettering. Volunteers are asked to do this work respectfully.

Lewis and colleague Deirdre Dragovich of the University of Sydney have begun working through two years' worth of data collected so far, and they are still looking for more.

With the data they have so far, the researchers are looking at weathering rates over time and at potential links with atmospheric changes. Specifically, Lewis is interested in seeing if periods of increased rain in particular areas accelerated weathering rates, and if the arrival of the Industrial Revolution ? and the increase in pollution that accompanied it ? are reflected in increased gravestone weathering, and how the weathering rate has changed since the Industrial Revolution.?

So far, they've seen that cemeteries in big cities seem to be weathering most rapidly, he said. This isn't a surprise since more acid rain-causing pollutants, particularly sulfur dioxide and nitrogen oxides, are released over urban areas.

Laura Guertin, an associate professor of earth science at Pennsylvania State University, already frequented the Cumberland Cemetery across the street from the Brandywine campus with her introductory geosciences students, when she began participating in the project in 2011.

At that cemetery and another in central Pennsylvania, Boalsburg Cemetery, she and her students have undertaken a wide range of projections, including comparing weathering rates of different types of stones (nearly all are granite or marble), and gleaning information about the history of the local community, such as how long people lived.?

"At first they are a little creeped out," Guertin said. "I tell them, 'Don't worry, I will bring you all back with me.'"

Her students found a weathering pattern they didn't expect in certain areas within Cumberland Cemetery, where they found stones with the most wear on the sides of the top of the stone, rather than at the middle point.

"This is something I want my students to look into," she said.

Vaccine Could Cut Heroin Addiction

I've been a herion addict and was on methadone and the Buprenorphine trials we had here for a bit. I was an herion addict for about 10 years, let it control my life, homeless and all that crap. Got on methadone on and off for some years, did the Bupernorphine trials, got back on methadone again.

Even though I wanted to quit using dope, i would always end up back because want wasn't enough. While I was on methadone last, I got to talk to a shrink, and after a bit, found out I was dyslexic and ADHD and other fun stuff that I should of found out before my mid 30's. It was getting help for my ADHD and dyslexia that made it so I was finally able to say Fuck you to herion, and even methadone. I volunteerly lowed my dose till it was down to nothing, and stopped going to the stupid ass methadone clinic. By that time, I hated it and the people that went there and mostly the policies they have there.

Making it so people can't feel the high of doing Herion is going to make them do other drugs so they can feel something. Plane and simple. I knew a girl who'd OD because she had one of those thingies they put in your stomache so you can't get high. She kept trying and you want to know whats up with her now? She talks to herself. Mentally, she's gone. Kept trying to get high. She didn't die from it, but she's no longer the person she was.

You want to get herion addicts to stop? You need to find the reason why they are a heroin addict and fix those. Me? I was confused, I didn't understand people and people didn't seem to understand me. Stress and frustration were my enemies, and they didn't matter if I was high. Unfortunetly, being a herion addict means you don't get high as much as you get well.

Anyways, I think this is the wrong approach big time. But it doesn't surprise me, people don't understand addiction, and that is why a lot of people get addicted to stuff.

An Alabama judge on Thursday acquitted the man accused of drowning his newlywed wife during a honeymoon diving trip to Australia eight years ago, ruling that prosecutors did not prove the man intentionally killed his wife to collect on a life insurance policy.

Circuit Judge Tommy Nail issued his ruling before the defense had even presented its case in the two-week-long trial and before jurors were given the case to deliberate.

Gabe Watson, 34, had faced life in prison without parole if convicted of murdering his wife, Tina Thomas Watson, in 2003. He already served 18 months in an Australian prison after pleading guilty there to a manslaughter charge involving negligence.

Nail agreed with defense arguments that prosecutors failed to show Watson intentionally killed the woman. Prosecutors claimed he drowned her for insurance money, but the only eyewitness testified he thought Watson was trying to save the woman.

The state's evidence was "sorely lacking" and did not prove Watson had any financial motive. "I don't think anyone knows for sure what happened in the water down there," Nail said.??

Defense attorneys had argued that Watson didn't stand to gain anything monetarily because Tina Watson's father was the beneficiary of her life insurance policy. They contended her death was an accident.

Gabe Watson's father, David, hugged his son in the courtroom after the judge made his ruling. He said every court that had looked at the case determined Gabe did not intentionally kill his wife.

"I'm just so relieved. Hopefully he can put his life back together," David Watson said.

Tina Watson's father, Tommy Thomas, had testified earlier in the day. He described how his family's grief and shock over Tina Watson's death turned to suspicion of Gabe Watson.?He testified that he got the news of his daughter's death not from Watson, but from the defendant's father more than a day later.

Prosecutor Don Valeska walked with his arm around Tommy Thomas, who appeared to be in shock and stunned by the judge's decision.

"It should have gone to the jury for them to decide," Thomas said of the judge's decision.

The Alabama Attorney General's Office had sought to have Watson convicted on charges that he planned his bride's death in Alabama, before the couple departed for their honeymoon.

Thomas had testified earlier that shortly after the death, his wife, Cindy, was worried about Gabe Watson's condition. However, evidence showed relations between Watson and his wife's family frayed quickly as the Thomases began having doubts about what happened and Gabe Watson began asking for Tina Watson's belongings.

Gabe Watson's father called to tell them about the woman's death more than 15 hours after she drowned, Thomas said, and Tina Watson's family never heard from Gabe Watson until they attempted to contact him through the U.S. consulate in Australia.

Thomas said that in a phone call from Australia, Watson claimed his wife gave him a thumbs-up underwater, indicating she wanted to go back to the surface. Watson said he was leading her back to a rope when she panicked, knocked off his mask and air hose, and began sinking, according to Thomas.

But during a later talk at a lawyer's office, Thomas said, Watson changed his story and said the woman indicated she wanted to go back to the rope leading to the top rather than go directly to the surface. Staring directly at Watson from the witness stand, Thomas said he asked his former son-in-law at that time: "When Tina gave him the thumbs up sign to go to the surface, why didn't he just take her to the surface?"

The judge blocked Thomas from testifying about Watson's alleged desire to increase the woman's life insurance policy, a blow for prosecutors who earlier had been barred from presenting other
evidence about Watson's actions after the death.

Faster-than-light neutrinos explained?

The faster-than-light neutrinos seen by the OPERA particle physics experiment last year may have just been explained. By a loose cable. I wish I was joking.

To back up a little, the OPERA collaboration based at the Gran Sasso laboratory underneath the mountain of the same name in Italy published a paper to pre-print server arxiv.org last September saying that they had seen neutrinos, a type of sub-atomic particle, travel faster than the speed of light. They recorded neutrinos, which had travelled from CERN, Geneva, through the Earth to Gran Sasso, Italy, arriving at the laboratory 60 nanoseconds faster than they would had they travelled at the speed of light.

Since then, scientists around the world have been collectively scratching their heads and publishing papers that tended to fall into one of two categories: suggesting an error with the experiment (such as the clocks at the two laboratories not being synchronised properly), or suggesting an addition to the current theory of particle interactions that could explain the strange result (for example, a new dimension that the neutrinos could have skipped through to make their journey shorter ? so they would have never actually travelled faster than light at any point).

But I don?t think anyone expected it to be something as simple as this.

Today, Science is reporting that a fibre optic cable connecting a GPS receiver and an electronic card in a computer was loose. They go on:

After tightening the connection and then measuring the time it takes data to travel the length of the fibre, researchers found that the data arrive 60 nanoseconds earlier than assumed

This news (though still unconfirmed) rather casts a shadow over another recent explanation, involving something slightly less ridiculous.

In a paper published in journal Astronomy & Astrophysics, Claudio Germana of the Astronomical Observatory of Padova, Italy, suggests that there was a problem with the synchronisation of clocks at the two ends of the experiment. His calculations suggest that if the experiment had been run at a different time of year, the neutrinos would in fact have arrived 50 nanoseconds later than light.

I spoke to Carlo Contaldi, a physicist at Imperial College London, who last year published a paper on arxiv.org pointing out a possible problem with clock synchronisation, about the new paper. Though he thought the calculations and the large effect the calculations seemed to show were ?interesting?, he had some reservations:

[Germana] does not seem to mention the latest measurements that were carried out by OPERA in November 2012. Those showed a consistent value for the neutrino?s time of flight as the previous results and it would be interesting to see how that time frame fits in with these corrections.

It?s an interesting hypothesis though ? and one that is easily testable by running the experiment at a different time of year.

This paper is just the latest in a long string of attempts to explain the faster-than-light neutrinos. For more of the explanations that have been offered over the last few months, have a look at a timeline I made that follows the story right from the beginning until now.

All of these papers could have been for nothing, of course, if the new report of a loose cable is true. It would be a little disappointing if this turns out to be the case. I?m going to reserve judgement for now, at least until the ?sources familiar with the experiment? become something a little more concrete.

I?ll be updating the above timeline as the story unfolds.

UPDATE 23rd Feb: The OPERA experiment has issued an official statement. Here it is in full:

The OPERA collaboration has informed its funding agencies and host laboratories that it has identified two possible effects that could have an influence on its neutrino timing measurement. These both require further tests with a short pulsed beam. If confirmed, one would increase the size of the measured effect, the other would diminish it. The first possible effect concerns an oscillator used to provide the time stamps for GPS synchronizations. It could have led to an overestimate of the neutrino?s time of flight. The second concerns the optical fibre connector that brings the external GPS signal to the OPERA master clock, which may not have been functioning correctly when the measurements were taken. If this is the case, it could have led to an underestimate of the time of flight of the neutrinos. The potential extent of these two effects is being studied by the OPERA collaboration. New measurements with short pulsed beams are scheduled for May.

So, there was a (possible) faulty cable that might have led to an underestimate of the time it took the neutrinos to reach Gran Sasso, which would led to an overestimate of their speed. But there was also another fault that might have led to an underestimate of the speed. Looks like we will have to wait for the new measurements in May to see just how much each of these faults contributed to the early arrival of the neutrinos and whether they can add up to the 60 nanoseconds to fully explain the result.

Hubble reveals a new class of extrasolar planet

ScienceDaily (Feb. 21, 2012) ? Observations by the NASA/ESA Hubble Space Telescope have come up with a new class of planet, a waterworld enshrouded by a thick, steamy atmosphere. It's smaller than Uranus but larger than Earth.

An international team of astronomers led by Zachory Berta of the Harvard-Smithsonian Center for Astrophysics (CfA) made the observations of the planet GJ 1214b.

"GJ 1214b is like no planet we know of," Berta said. "A huge fraction of its mass is made up of water."

The ground-based MEarth Project, led by CfA's David Charbonneau, discovered GJ 1214b in 2009. This super-Earth is about 2.7 times Earth's diameter and weighs almost seven times as much. It orbits a red-dwarf star every 38 hours at a distance of 2 million kilometres, giving it an estimated temperature of 230 degrees Celsius.

In 2010, CfA scientist Jacob Bean and colleagues reported that they had measured the atmosphere of GJ 1214b, finding it likely that it was composed mainly of water. However, their observations could also be explained by the presence of a planet-enshrouding haze in GJ 1214b's atmosphere.

Berta and his co-authors, who include Derek Homeier of ENS Lyon, France, used Hubble's Wide Field Camera 3 (WFC3) to study GJ 1214b when it crossed in front of its host star. During such a transit, the star's light is filtered through the planet's atmosphere, giving clues to the mix of gases.

"We're using Hubble to measure the infrared colour of sunset on this world," Berta explained.

Hazes are more transparent to infrared light than to visible light, so the Hubble observations help to tell the difference between a steamy and a hazy atmosphere.

They found the spectrum of GJ 1214b to be featureless over a wide range of wavelengths, or colours. The atmospheric model most consistent with the Hubble data is a dense atmosphere of water vapour.

"The Hubble measurements really tip the balance in favour of a steamy atmosphere," Berta said.

Since the planet's mass and size are known, astronomers can calculate the density, of only about 2 grams per cubic centimetre. Water has a density of 1 gram per cubic centimetre, while Earth's average density is 5.5 grams per cubic centimetre. This suggests that GJ 1214b has much more water than Earth does, and much less rock.

As a result, the internal structure of GJ 1214b would be extraordinarily different from that of our world.

"The high temperatures and high pressures would form exotic materials like 'hot ice' or 'superfluid water', substances that are completely alien to our everyday experience," Berta said.

Theorists expect that GJ 1214b formed further out from its star, where water ice was plentiful, and migrated inward early in the system's history. In the process, it would have passed through the star's habitable zone, where surface temperatures would be similar to Earth's. How long it lingered there is unknown.

GJ 1214b is located in the constellation of Ophiuchus (The Serpent Bearer), and just 40 light-years from Earth. Therefore, it's a prime candidate for study by the NASA/ESA/CSA James Webb Space Telescope, planned for launch later this decade.
